The operator problem
Most AI intake systems optimize for speed and then quietly destroy provenance. Requests arrive through chat, email, forms, and meetings; the generated answer survives, while the original evidence and decision owner disappear.
The operating pattern
Capture the original request as an immutable record. Attach every source used by the model. Generate a proposed action package, not an automatic action. Route that package to a named approver and record the exact approved version.
Minimum controls
Store capture time, source URL, claim-level evidence, content version, approval identity, release time, and downstream status. A correction should create a new version instead of overwriting the record that was originally approved.

Pick one recurring intake process this week. Add a source ledger and approval state before adding more automation. If the workflow cannot explain why it acted, it is not ready to act.
